The Annual Physician Block Fee Is Effective Immediately 

 

The Ministry of Health pays physicians directly for most health services but there are more and more services that are not covered by the Ministry.  We don’t like to charge you for services provided outside of your scheduled visits and you don’t want to pay every time you need one.  To make it easier for you, we offer a Block Fee to cover a host of services you need every year.

 

The “Block Fee” covers uninsured services on an annual basis including renewals of prescriptions without a medical appointment, and auto-faxed requests generated by your pharmacy. It also covers a host of other services, included with this letter there is a list of items covered by the Block Fee. Please see the full list of services covered attached.

Our Office Annual Physician’s Block Fee is:

Per Person  $150.00

Unemployed or on disability $100.00

Couple’s Package    $250.00

Family Package       $350.00

 

You can also have the option to pay for individual services at the time a service is provided and not pay the Block Fee. 

 

If you would like a copy of your complete digital medical records, you may order them at any time from the office for $350.00 and up, this process may take 2 to 4 weeks to be completed.

 

To pay for the Annual Block Fee using a debit or credit card, please call our office or send a cheque by mail to our office.

No to the Walk-in Clinics

 

Walk-in Clinics do not cooperate with or provide information about your visit to your family doctor. PLEASE DO NOT USE WALK-IN CLINCS.

 

When you need medical services outside of appointment times, PLEASE Call, Email or TEXT the office and we will book you immediately in a timely fashion.  If you have an emergency and can’t get through our office, please go to your nearest emergency hospital. Do not go to walk in clinics.

 

We have 24 hours text messaging.  We will respond to your texted questions and/or set up an appointment for you within 24 hours.  If you use walk in clinics, we do not get your medical reports for the visit and lose the continuity of your medical information so important for your care.

 

If you have used walk in clinics in the past, please ask the clinics for a copy of your visit medical record and forward it to us.  Please also fill in the Ministry of Health Enrolment form to comply with our office policy, please see our office policy in the attachment.

Please make sure to have a valid OHIP card and please provide our office with your new OHIP card version code upon renewal immediately.
