RE: Block Fee services / Emergency patient services
The Ministry of Health pays physicians directly for most health services but more and more services are not covered by the Ministry. Therefore you have a choice of two options:
You may pay for individual services at the time they are provided, or you may choose to pay an annual block fee.
The “Block Fee” covers uninsured services on an annual basis such as renewals of prescriptions without a medical appointment, auto-faxed requests from your pharmacy, and 24/7 text messaging to our office. It also covers a host of other services. For a complete list of such services see the attached document below.

Please note that the Annual Physician Block Fee is due every September, and will be automatically deducted each year.
Our Office Annual Physician’s Block Fee is:
Per Person $200.00
Unemployed or on disability $150.00
Couple’s Package $350.00
Family Package $500.00
Annual Block Fee can be paid via debit or credit card.
Please note all patients must have a credit card or debit card on file to avoid any interruption of services. In the event that your card has expired or been replaced please update your information immediately.
Requests for Digital Medical Records
If you would like a copy of your complete digital medical records, you may order them at any time from the office for $450.00 and up, this process may take 2 to 6 weeks to be completed.
No to the Walk-in Clinics
PLEASE DO NOT USE WALK-IN CLINCS. Walk-in Clinics do not cooperate with or provide information about your visit to your family doctor. If you use walk in clinics, we do not get your medical reports for the visit and lose the continuity of your medical information which is so important for your care.
When you need medical services outside of appointment times, please call, email or text the office and we will book you in a timely fashion. If you have an emergency and can’t reach our office, please go to your nearest hospital emergency department.
We have 24 hours text messaging. We will respond to your texted questions and/or set up an appointment for you within 24 hours. If you have used a walk in clinic in the past, please ask the clinic for a copy of your medical record of your visit and forward it to us. Please also fill in the Ministry of Health Enrolment form to comply with our office policy. To see our office policy click here

Please make sure that you have a valid OHIP card. If you have recently renewed your OHIP card, please provide us with the new version code (the version code is the 2 letters to the right of your OHIP number on your card).
